Completing the Self-Assessment Survey

Description

This survey examines the status and need for improvement of four behavior support systems: (a) school-wide, (b) non-classroom, (c) classroom, and (d) individual students. A summary of the survey results is used to develop an action plan for implementing and sustaining Positive Behavioral Interventions and Supports (PBIS) systems throughout the school.

The survey usually takes approximately 40 minutes to complete and is conducted annually (see measurement schedule for more information) by the school staff.

Completing the Self-Assessment Survey

To complete and submit a Self-Assessment Survey, follow these steps:

  1. Go to the PBIS Surveys Web site (www.pbssurveys.org).
  2. Login as a respondent using the account number for your school.
  3. From the Surveys menu item select Self-Assessment -- Complete Survey. If your school is currently conducting a Self-Assessment Survey, first page of the the survey will display. (If your school is not currently conducting a Self-Assessment, a page with a message informing you of this will be shown.)
  4. Select your occupation or role from the drop-down list.
  5. The Self-Assessment Survey has four sections, one for each Positive Behavioral Interventions and Supports system: school-wide, non-classroom, classroom, and individual. Questions about each system appear on a separate page, beginning with the schoolwide system. You can navigate forward and backward through the survey form pages by clicking on the Next Section and Previous Section links that appear at the top and bottom of the survey area of each page.
  6. Answer all the questions you can in each section. If you cannot answer a question, leave it blank (do not select any value).
  7. When the last section, "Individual Student Systems", is completed, click on the Submit button at the top or bottom of the survey area of the page.

Generating Self-Assessment Survey Reports

The reports and charts summarize responses by staff to the Self-Assessment Survey at a school. Some reports depict the status of PBIS systems at a given time, while others allow you to make comparisons across time. The reports are used to:

  • Determine the status of PBIS in a school.
  • Guide the development of an action plan for improving PBIS.

The resulting action plan can be focused on any combination of one or more of the four PBIS system areas (school-wide, nonclassroom, classroom, individual).
