Description
This checklist is designed to assist school teams in monitoring School-wide Positive Behavioral Interventions and Supports (SW-PBIS) implementation activities, and to guide action planning of SW-PBIS in the school.
The MiBLSi school team should complete both the Start-Up Activity and On-Going Activity Monitoring checklists each quarter (see measurement schedule for more information) to monitor activities for implementation of PBIS in the school. Only one checklist is completed quarterly by the team and submitted online at the PBS Surveys Web site. The Team Implementation Checklist usually takes about 10 minutes to complete.
Completing the Team Implementation Checklist
To complete and submit a Team Implementation Checklist, follow these steps:
- Go to the PBSsurveys Web site (www.pbssurveys.org).
- Login as a respondent using the account number for your school.
- From the Surveys menu item select Team Checklist -- Complete Survey. If your school is currently conducting a Team Checklist, the survey form page will display. (If your school is not currently conducting a Team Checklist, a page with a message informing you of this will be shown.)
- Optionally, select your name from the drop-down list. If your name does not appear on the staff list, you may add it using the link that appears immediately blow the drop-down.
- The Team Checklist consists of two sections: Start-up Activities and On-Going Activities. Respondents must answer all questions in one or both of these sections. When the page first displays, the Start-up Activities tab is on top. To switch sections, simply click on the On-Going Activities tab.
- Each section has a link to explain the achieved criteria for each item in the section. Click on the blue box More About These Items to display this information.
- When you have answered all questions in one or both sections, click on the top or bottom Submit buttons. You have completed the Team Checklist.
Generating Team Implementation Checklist Reports
Two reports are available for the Team Checklist surveys.
- Implementation by Feature (shows the percentage implemented and partially implemented for 9 separate categories related to Universal PBIS implementation).
- Overall Implementation (shows overall scores as (a) the percentage of items fully implemented and partially implemented and (b) the percentage of implementation points. The report displays one row of this data for each Checklist in ascending date order).
